View/General Use. You are not required to register or provide us with any personal information in order to view information on our site.

Information Service. If you decide to sign up for our "service," you will be asked to provide certain information so that we may contact you. We will ask you to provide your name, address, telephone and fax numbers, e-mail address, social security number, date of birth, Medicare number, primary and secondary insurance carrier information, physician contact information (including name, city, state, zip code, telephone and fax numbers), your gender and health-related information. Reorder Service. If you are a current customer and have previously placed an order with Sirius through our 1(800) toll free telephone order service, you may request a refill of your order through our site. You will be asked to enter your 5-digit customer number and zip code. This information is only collected if you choose to reorder your products through our site.

Information Use and Disclosure

The information that you share with us, either to sign up for our information service or to reorder products, will only be used by the Procyon Corp. Companies to evaluate your needs so that we may provide you with information that is most useful and relevant to you.
